Downloading Periodic Assessment History

You can download the Periodic Assessment history, in Excel format, for the life of an incident or for an individual decision from the Periodic Assessment tab.

To download Periodic Assessment History for the life of an incident:

1. Navigate to the Periodic Assessment tab.
2. Click the Excel icon located in the upper left of the Periodic Assessment List.

To Download Periodic Assessment History for an individual decision:

1. Navigate to the Periodic Assessment tab.
2. Select a decision.
3. Click View Info. Locate the Assessment History near the bottom of the screen.
4. Click the Excel icon located in the upper left of the screen.
